If you’ve gone so far as to have finished the premedical course requirements (biology, physics, chemistry, and organic chemistry), taken the MCAT, and have a good GPA you could very well apply now. The MCAT is below the average for matriculated students, however.
The more important question in the mind of an admissions officer would be: Does dannyc know what physicians do day in and day out? Does he have the necessary clinical experience to show us that?
Taking the PharmD versus not is really an issue of money. It won’t make much of a difference to the med school AdComs whether or not you have an advanced degree.
The process of getting in requires having a good story to tell, and for most successful premed applicants that story begins in the first year of college.
